Click on 'OS Type', 'Area' and 'Priority' buttons to define your request. Other sections should be filled in as appropriate. Once you have completed the form, click on the send option at the bottom and it will be placed on the appropriate request list.
Once you have sent the request you will be given a request ID number which will remain with that job until it is resolved, and a link with which to access the main list pages that show all requests and their current status.
If you wish to check up on, or add any comments to your request at a later stage, access the request form via the above URL and select the 'keep track and follow up' link located in the text paragraph. Once the list appears, IT-Support will be the default, click on the ID number or subject line associated with your request.
